Optic Nerve: Clinical Examination
نویسنده
چکیده
Optic nerve-head examination is probably the most important step in the diagnosis of glaucoma and is also extremely important in monitoring patients with established glaucoma. There are several ways to clinically examine the optic nerve head, including direct ophthalmoscopy, indirect ophthalmoscopy, and slit lamp biomicroscopy with contact lenses (such as a Goldman lens), handheld lenses (such as a 78 or 90-diopter lens) or the Hruby lens. The advantages of slit lamp biomicroscopy, the preferred method for optic nerve evaluation, over the other methods mentioned are the quality of the stereopsis and magnifi cation provided. Although slit lamp biomicroscopy with handheld lenses can be performed through an undilated pupil, a stereoscopic view may be possible only if the pupil is dilated. In addition to slit lamp examination, optic disc stereophotography provides complimentary clinical information. For example, data from the Ocular Hypertension Treatment Study (OHTS) show that 84% of 128 cases of optic disc hemorrhages were detected on disc photographs and not on the clinical exam [1] . Clinical examination of the optic nerve should be performed with similar methodology each and every time it is executed, in order not to miss important aspects of the examination. In my view, examination of the optic nerve head should start with an evaluation of optic disc size, since disc size is extremely important in the interpretation of other optic nerve fi ndings. Even a simple subjective assessment of whether the disc is small, large, or average in size without specifi c measurements can be of value. The exam should then proceed to a careful assessment of the neuroretinal rim, looking for areas of thinning, notching, nasal cupping, M. T. Clinical Applications of Optical Coherence Tomography in Ophthalmology
Assessment of the peripapillary nerve fiber layer and macular thickness can be determined in ophthalmology using optical coherence tomography (OCT). Decreased nerve fiber layer thickness and macular ganglion cell thickness in optic nerve ischemia have been correlated with visual field loss. OCT allows deep optic nerve head evaluation which helps understand pathophysiology of diseases. Furtherm...
متن کاملEvaluating Optic Nerve Damage: Pearls and Pitfalls
Primary open-angle glaucoma is a progressive optic neuropathy involving loss of retinal ganglion cells and their axons at the level of the optic nerve head. This change manifests as thinning and excavation of the neural tissues and nerve fiber layer. Therefore, it has long been known that the structural appearance of the optic nerve head is paramount to both glaucoma diagnosis and to the detect...

Background :There is not any data available about the effect of high bar pressure condition on intracranial pressure. In this study, the effect of diving on the optic nerve and sheath diameters as non-invasive markers of intracranial pressure has been investigated. Methods : Twenty professional male divers from twenty one volunteers were chosen for this cross-sectional study. Only one perso...

متن کاملClinical Optic Disc Evaluation in Glaucoma
Examination of the optic nerve head (ONH) is essential for the diagnosis of glaucoma and assessment of its progression. Slit-lamp biomicroscopy with a handheld lens is the best method of ONH examination since it provides good stereopsis and magnification. ONH stereo photographs are complementary and may identify findings missed on slit-lamp examination. As a result of its subjective nature, a s...
متن کاملProstatic carcinoma metastatic to the optic nerve.
Optic nerve can be infiltrated with various tumors and inflammatory processes, with a considerable prevalence of primary over secondary tumors. Metastases of gastric carcinoma to the optic nerve are less frequently, and those of prostatic carcinoma very infrequently observed. A 66-year-old man presented with metastasis to the optic nerve with consequential vision loss to the level of light perc...
